Service Description

The Habilitation Service supports children and young people with vision impairment to develop independence, mobility, and life skills.

The service works with children and young people aged 0–25 to promote confidence, functional skills, and participation at home, in education, and in the community.

Support is personalised and outcomes focused, in line with Education, Health and Care Plans (EHCPs).

Independent living skills (e.g. dressing, personal care, money skills)

Access and navigation skills for home, school, and community settings

Support with communication and social interaction

Joint sessions with parents/carers to model strategies and techniques

Information on vision impairment, mobility, and daily living skills

Transition support for young people moving into adulthood

Liaison with schools, colleges, health professionals, and other services
