Service Description

Tower Hamlets GP Care Group provides health services and programmes for children, young people, and families. Our teams deliver health visiting, school health, and family nurse support, as well as specialist programmes to promote wellbeing and early intervention. Our services include:

Health Visiting – Healthy Child Programme for ages 0–5, including antenatal contact, new birth visit, 6–8-week check, 3–4-month check, and 1- and 2-year reviews
MECHS Programme – home visits, child health clinics, breastfeeding support, perinatal mental health, safeguarding, signposting, and parenting support
School Health – Healthy Child Programme for ages 5–19, vision and hearing screening, NCMP checks, coffee mornings, drop-ins, and individual support
Family Nurse Partnership – structured home visits from pregnancy until the child is aged 1–2 years, delivered by specially trained nurses using a strength-based approach

Start for Life Workforce Pilot (Bright Beginnings):

Perinatal mental health
Immunisation plus advice on feeding, starting solids, and vaccinations
Social communication support for ages 10–15 months
Social prescribing for ages 0–2 years
Contacts take place in homes, Best Start Family Hubs, libraries, parks, and community groups.
