Service Description

The CCNT Service provides nursing care and treatment to children aged 0 – 19 years of age, supporting families to meet their care needs. Enabling children and young people to achieve their optimum health and reduce the impact of illness on their health and wellbeing in their own home or other community settings (i.e., schools and nurseries).

We aim to facilitate early discharge from hospital, and prevent repeat attendances to A & E., and we work closely with other health professionals to support the delivery of coordinated care.

Bank holidays 10am to 4pm.

We also deliver our services from Children's Centres and Nurseries.
