This is a specialist community based service for teens and young adults with disabilities. The service offers support during the time of transition for young people from Children’s to Adult Services. Transition will aim to make the changes that individual young people want to see such as getting into employment, finding a place to live and being more independent. Transition workers with various backgrounds are able to provide advice and support in key areas such as education, employment, housing and sport and leisure.
